Kindergarten Registration
Please bring with you when you return the completed packet:
- Birth Certificate (Certified copy)
- Current Immunization Record
- Custody Papers (if applicable)
- Social Security Card
You must provide two (2) proofs of residency from the following list:
- Ohio Works First identification card with district address
- Ohio Drivers License with your current address in the district
- Certified voter registration document with district address
- Utility bill in your name (gas or electric) or copy of utility deposit
We will not register any child without above documentation.
